Step 1

Start by filling out the brief request form for a free 15 minute initial screening with one of our providers.

Step 2

You will receive a link in your email to choose a time slot for your initial screening appointment.

Step 3

During the screening appointment the doctor will help you decide whether you will benefit from establishing care with ZOYA clinic. If you decided to proceed you will receive intake paperwork in your email and you will be scheduled for a 60 minute intake as a new patient.

Insurance

ZOYA is a private pay practice. We are not in contract with any insurance company at the moment. You will be provided with a super bill after each session and will most likely be reimbursed for a portion of your visit cost afterwards per your insurance benefits.

Adult patient: New evaluation visit

60 minutes - $400

In this comprehensive evaluation information about your symptoms, history, and overall mental health is gathered to make an accurate diagnosis and recommend appropriate treatment options.

Child/adolescent patient: New evaluation visit

75 minutes - $475

In this comprehensive evaluation information about your child's symptoms, history, and overall mental health is gathered to make an accurate diagnosis and recommend appropriate treatment options.

Adult patient: Follow up medication management visit

25 minutes - $245

For established patients only. The provider will check in on your progress since your last visit and adjust any treatment plans as needed.

Child/adolescent patient: Follow up medication management visit

25 minutes - $275

For established patients only. The provider will check in on your child's progress since your last visit and adjust any treatment plans as needed.

Psychotherapy visit

50 minutes - $275

For both new and established adult and youth clients.
